Q: Is it possible to get to know Doctor Tomchik or Penny Cuoco before I choose him or her as my Primary Care Physician (PCP)?

A: Yes. Doctor Tomchik and Penny Cuoco pride themselves on getting to know his and her patients. Please call ahead and our staff will arrange for Doctor Tomchik or Penny Cuoco to spend a few minutes introducing him or herself and answering any questions you may have.

Q: How do I know if you accept my insurance?

A: The best way is to call your insurance company before you schedule your first visit with Doctor Tomchik to ensure that your visit will be covered by your insurance company.

Q: If I get injured while working, do you accept worker's compensation?

A: Yes, but make sure you have the following items before scheduling an appointment: approval from your case manager to see Doctor Tomchik, your claim number, the name and phone number of your case manager, and a billing address for your worker's compensation.

Q: Do you see young children?

A: We see all children ages 2 and up. If you would like for Doctor Tomchik to see a child under 2 years old instead of a pediatrician, that can be arranged.

A: We accept cash, personal checks, money orders, travelers' checks, Visa, Master Card, and American Express; unfortunately, we do not accept Discover.

Q: Is your office wheelchair accessible?

A: Yes.

A: You can download, fill out, and print out the necessary forms here, or you can request them by calling our office.

A: Yes, if it is your first visit to our office. If you are required to change your PCP (Primary Care Physician) please do so before you make an appointment.

Q: Do you see patients without medical insurance?

A: Yes.

Q: Do you speak languages other than English? (¿Ustedes hablan español?)

A: Yes. Our staff speaks both Spanish (se habla español) and Creole (nou pale creole).

A: Unfortunately the time spent waiting depends upon many factors, including: time of year, day of the week, and time of day, and more. However, we do set the goal of ensuring that each and every patients who made an appointment sees the doctor within one hour of their appointment.

Q: What if I am sick and I don't have an appointment, can I come in without an appointment?

A: Yes, we accept walk-in's.

Q: What services are available over the phone?

A: Scheduling and canceling appointments, requesting refills for medications, requesting a referral to a specialist, receiving test results, and paying outstanding bills.

Q: What if I need refills on my prescription(s) before appointments?

A: If you need prescription refills before your appointment please contact your pharmacy and ask them to fax us a prescription refill request. Our fax number is (954) 450-3557. For more information please visit our requests page.

A: Yes, we offer laser hair removal, skin rejuvenation, a weight loss program, criminal justice physicals, physician administered vitamins and supplements, stress tests, radiowave treatments, INS (immigration and naturalization) physicians, free school immunizations, school and sports physicals, and more. Q: I need to see a specialist, can I make my own appointment?

A: It depends upon your insurance plan. Preferred Provider Organizations (PPOs) typically allow for patients to make their own appointments to see specialists; however, Health Maintenance Organizations (HMOs) require prior authorization from your Primary Care Physician (PCP). The only way to know for sure is to contact your insurance company.

Q: How do I make appointments with the chiropractor and/or podiatrist?

A: If you need an appointment with Doctor Alan Himmel, the chiropractor, please call him at: (954) 659-8600 in Broward, and (305) 979-5549 in Dade. Please feel free to visit Doctor Himmel's web site at: www.miramarchiropractor.com.

Q: What is a deductible? What is a co-pay? What is co-insurance?

A: A deductible is an amount of money set by your insurance company that you must pay before the insurance company begins paying for your visits.

A co-pay is a set amount of money that you must pay each and every time you visit the doctor.

Co-insurance is a percentage of the doctor's fee that the patient is responsible to pay. Co-insurance differs from co-pays in that a co-pay is always the same amount of money, but the amount of co-insurance differs depending on what services were provided at the doctor's office.

Q: I just received a bill from your office that I thought would be covered by insurance, what do I do now?

A: Please contact your insurance and request an Explanation of Benefits (EOB). Insurance companies often will not pay for issues as simple as forgetting to update your address.

A: Call First Federal Credit and Collections (their phone number will be found on the collections letter).

A: 45 days after the first statement from First Federal Credit and Collections.

A: Call Quest Diagnostics (the phone number can be found on the bill) and find out why you received a bill.
