Prescription Refill Requests

In order to request a prescription refill please contact your pharmacy and ask them to fax a refill request to Robert S. Tomchik, M.D.'s office at (954) 450-3557. Once we have received the refill request by fax we will follow-up with you letting you know that either your prescription has been refilled or that you need to make an appointment to see Doctor Tomchik or Penny before we can process the refill request.

Most pharmacies have an option in which you can request refills on-line; however, we cannot guarantee that they will automatically fax us a refill request, and without a faxed refill request we will be unable to process your refill.
